ViroMed has special test request forms designed specifically for use by tissue bank clients.
Test request forms are revised as new testing services are added to ViroMed's test menu. For specialized testing or new tests not preprinted on the forms, clients may simply write in their requests in the spaces provided.

Supplies for collection and transportation of donor samples being shipped to ViroMed can be ordered by calling ViroMed Account Management or by faxing a completed donor supply request form to ViroMed Account Management at 336-436-1812.
